Hours/Referral
The office is open Monday through Friday from 7:45 A.M. to 5:00 P.M. with referral by your Pediatrician being necessary. An appointment for a routine evaluation can usually be obtained within one to two weeks. Urgent appointments can be obtained within a day; however, your Pediatrician will need to call the office directly to coordinate this.
What To Bring
- New patients should arrive at the office at least 15 minutes prior to their appointment in order to complete the New Patient History, Patient Registration and Consent forms. To save you time, these forms can be downloaded and completed prior to your child's appointment.
- Please bring your current insurance card and photo ID.
- Non-parent guardians must bring a current Power Of Attorney.
Hearing Problems
If you think your child has a hearing-related problem or may need a hearing test, please notify the receptionist when you schedule your appointment. A separate appointment with the Audiologist can be arranged.
